The ABCs of behavior management refer to the antecedent, behavior and consequence. A child engages in an activity that causes a behavior to occur, resulting in a consequence.
Examples of Behaviors to Report
The behavior could be a positive or negative reaction to the antecedent. Likewise, the positive or negative consequence that results should directly relate to the behavior that occurred. As a teacher, you must compose a behavior report that can accurately explain the ABCs in a situation.
Create a behavior report template that addresses the ABCs of behaviors. Divide the template into three sections that correspond to the ABCs and one section that allows for brief brainstorming of an alternative to the behavior under scrutiny.
Use desktop publishing or word processing software to create the template so you can easily edit it during the school year. Make the template student-friendly for the option to give students responsibility for documenting their behaviors.
Label the first section with an appropriate title such as "Observed Behavior" or "What Happened. Describe a behavior exactly as it occurred without adding opinions or assumptions. Make a checklist of the acknowledged classroom and school rules and arrange them in the second section. Monitor a student's response to make sure he understands the correlation between his behavior and the broken rule.

It is the first step toward creating a Behavior Improvement Plan, to support the students success. This sample requires the teacher to sign per day or per half day each time the child exhibits appropriate behavior.
There should be a reinforcer or reward in place for a specific number of teacher initials. This plan requires the reinforcers and consequences to be listed. Initially the teacher should stand next to the student and administer it, but after a day or two, the student should be ready to take over.
You may want a peer you trust to monitor the other student. This works well with young elementary students, but with fourth or fifth grade students, in which case a teacher should be leery of opening a compliant student up to bullying on the playground, etc.
This worksheet is more flexible, since unlike the above printable, this form is blank. You could use a different Behavior for your countdown on consecutive days, alternate, or take a more flexible approach.
You need to start with a single behavior to start, and add behaviors as you go. This might be part of a two pronged approach, as you may want to use the countdown for one behavior, while focusing on other behaviors with a behavior contract. In other words, you are challenging the student to prove that he has mastered the calling out behavior, or the talking during instruction behavior.

He shows me nothing but disrespect and is making it impossible to teach my class. John has stalked into class three times since the term started, late each time. He talks on his cell phone, or talks to others when he walks through the door.
One time he walked right past me and flaunted his late arrival in an attempt to insult me to in front of the entire class. This last time he was late I did try to talk to him after class, but he would have none of it.
He stood over me and tried to intimidate me, but I refused to be cowed. He should be removed from my class and if he is acting the same was in other classes, he should be removed from them too.
I am writing about John Doe, who is a student in my Writing class. I am concerned about repeated behavior on his part which is disruptive to my class.
Back in January when the term started John entered my class at least 20 minutes late 1 time during the first week of classes and two times during the second week of classes the class meets three times per week. The first time he walked into class late January 7 he was talking on his cell phone and finished his call just as he sat down in a chair in the back of the classroom.
When he entered I had to stop lecturing and the entire class followed John as he walked to the back of the room. He was smiling when he sat down. I asked the class to return their attention to me.
The next week John entered class about twenty minutes late on Monday, January He opened the door and stood in the doorway continuing a conversation with someone on the hallway.
It lasted about 30 seconds and then he said goodbye to that person and walked into the room, walked to the back and sat. School psychologists, special education teachers or another teacher may observe student behavior and document their progress. Student observations provide meaningful feedback for teachers by identifying student progress and the level at which a student is performing.
Observations are designed to hold students and educators accountable for the learning process. The information gathered and presented in the observation report includes statements about how a student performs both academically and behaviorally during direct instruction time and transition time. It may also include anecdotes about how a student interacts with his peers. These reports are usually read by psychologists, pediatricians, school specialists such as a speech pathologist or occupational therapist and behavior intervention specialists.
Parents, teachers and other school staff may also receive copies of these reports. Student observation reports include a title, name of the observer, name of student, demographic information about the student, reason for the observation and place and time of the observation. A general overview is included to set the context of the observation. Details about the environment and who is in the classroom are included. State the activities that occurred during the observation.

For example, the report should indicate if the student is a member of a dislocated family or English is the second language. Explain learning disabilities, anxiety triggers and intellectual gifts in the report. Behavior-based issues with the student are documented in this section as well. Document any support services the student receives. Benchmark percentages may be included in the report.
The report should include an assessment area with what the observer saw during the observation. There should be an observation and a comment section for each aspect of the observation. The aspects assessed are determined by the purpose of the observation. For example, if the student is being observed to determine if they are developing appropriately in the classroom, the report would address social and emotional behavior and motor skill demonstrations during the observation.
Other reasons for a student observations includes assessing age-appropriate behavior, proper motor skill development, speech and language development or issues with visual and hearing impairment. These types of observations can be performed by speech and language pathologists, visual specialists, hearing specialists, behavior interventionists or occupational therapists.
